WHAT TO EXPECT ON YOUR FIRST VISIT
Here at North Harbor Physical Therapy we strive for comfort and to maximize our time with our patients. For your initial visit please dress comfortably and arrive 15 minutes prior to your scheduled appointment time to ensure the completion of all paperwork. For your convenience our patient intake forms are available for download below so we can assist you even faster.
On the day of your visit please have:
- Insurance card and photo identification
- Referral**
- Any question you may have for your Physical Therapist
- A current list of medications
Our welcoming front desk staff is here to assist in any way. Please do not hesitate to ask any questions about billing, insurance, scheduling, etc. We look forward to welcoming you to our family and assisting you on your path to recovery.
